A safeguard for Medicare beneficiaries has become a way for drugmakers to get paid billions of dollars for pricey medications at taxpayer expense. The cost of Medicare’s “catastrophic” prescription coverage jumped by 85 percent in three years, from $27.7 billion in 2013 to $51.3 billion in 2015. Out of some 2,750 drugs covered by Medicare’s Part D benefit, two pills for hepatitis C infection - Harvoni and Sovaldi - accounted for nearly $7.5 billion in catastrophic drug costs in 2015. Medicare’s catastrophic coverage was originally designed to protect seniors with multiple chronic conditions from the cumulatively high costs of taking many different pills. Beneficiaries pay 5 percent after they have spent $4,850 of their own money. With some drugs now costing more than $1,000 per pill, that threshold can be crossed quickly. Lawmakers who created Part D in 2003 also hoped added protection would entice insurers to participate in the program. Medicare pays 80 percent of the cost of drugs above a catastrophic threshold that combines spending by the beneficiary and the insurer. That means taxpayers, not insurers, bear the exposure for the most expensive patients. Catastrophic spending accounts for a fast-growing share of Medicare’s drug costs, which totaled nearly $137 billion in 2015. The catastrophic share was 37 percent, yet only about 9 percent of beneficiaries reached the threshold for such costs. Catastrophic coverage will soon cost as much as the entire prescription program did when it launched. Experts say the rapid rise in spending for pricey drugs threatens to make the popular prescription benefit financially unsustainable.

Something strange is going on in medicine. Major diseases, like colon cancer, dementia and heart disease, are waning in wealthy countries, and improved diagnosis and treatment cannot fully explain it. Scientists marvel at this good news, a medical mystery of the best sort. The leading killers are still the leading killers - cancer, heart disease, stroke - but they are occurring later in life, and people in general are living longer in good health. Colon cancer is the latest conundrum. While the overall cancer death rate has been declining since the early 1990s, the plunge in colon cancer deaths is especially perplexing: The rate has fallen by nearly 50 percent since its peak in the 1980s. [Dr. Steven R. Cummings of the California Pacific Medical Center Research Institute], intrigued by the waning of disease, has a provocative idea for further investigation. He starts with two observations: Rates of disease after disease are dropping. Even the rate of “all-cause mortality,” which lumps together chronic diseases, is falling. In a national investigation, The Atlanta Journal-Constitution examined documents that described disturbing acts of physician sexual abuse in every state. Rapes by OB/GYNs, seductions by psychiatrists, fondling by anesthesiologists and ophthalmologists, and molestations by pediatricians and radiologists. A few physicians — with hundreds of victims — are among the nation’s worst sex offenders. The Roman Catholic Church, the military, the Boy Scouts, colleges and universities ... have all withered under the spotlight of sexual misconduct scandals and promised that abuse will no longer be swept under the rug. The medical profession, however, has never taken on sexual misconduct as a significant priority. And layer upon layer of secrecy makes it nearly impossible for the public, or even the medical community itself, to know the extent of physician sexual abuse. The AJC launched its national investigation a year ago after reaching a surprising finding in Georgia: two-thirds of the doctors disciplined in the state for sexual misconduct were permitted to practice again. Some states are apparently more forgiving than others when disciplining doctors in sexual misconduct cases. Georgia and Kansas, for example, allowed two of every three doctors publicly disciplined for sexual misconduct to return to practice. In Minnesota, it was four of every five. Nationwide, the AJC found that of the 2,400 doctors publicly disciplined for sexual misconduct, half still have active medical licenses today.

In public, Louis William Bair was brilliant, warm and engaging. In private ... women would later tell of groping, of vulgar comments and of aggressive, closed-door sex in his office. Bair was a doctor, and the women were his patients. Sexual contact between doctors and patients in Colorado, as in other states, is prohibited. But when Bair drew the attention of the Colorado Medical Board in 2002, it wasn’t because of violations. It was because the governor chose him to serve on the board, where he could help judge disciplinary cases for other physicians. Bair’s dual existence illustrates one of the most surprising findings of an Atlanta Journal-Constitution investigation of sexual misconduct by doctors. Among those found to have sexually abused patients are some of the most accomplished and admired – revered, even – physicians in the country. Their violations range from subjecting patients to lewd remarks ... to rape. Longstanding financial management challenges prevent the Army from issuing auditable financial statements. Army and Defense Finance and Accounting Service (DFAS) Indianapolis personnel did not adequately support $2.8 trillion in third quarter adjustments and $6.5 trillion in yearend adjustments made to Army General Fund data during FY 2015 financial statement compilation. In addition, DFAS Indianapolis did not document or support why the Defense Departmental Reporting System Budgetary (DDRS-B) ... removed at least 16,513 of 1.3 million records during third quarter FY 2015. DFAS Indianapolis did not have detailed documentation ... or have accurate or complete system reports. As a result, the data used to prepare the FY 2015 AGF third quarter and yearend financial statements were unreliable and lacked an adequate audit trail. Until the Army and DFAS Indianapolis correct these control deficiencies, there is considerable risk that AGF financial statements will be materially misstated and the Army will not achieve audit readiness by the congressionally mandated deadline of September 30, 2017.

The top 1% of Americans are finally recovering from the great recession. A new analysis of IRS data revealed that the average income of the top 1% of income earners grew by 7.7% in 2015, reaching $1.36m. Report author Emmanuel Saez, an economics professor at the University of California-Berkeley ... revealed that in 2015, the rich were also taking home larger chunk of the US income. “The share of income going to the top 10% of income earners – those making on average about $300,000 a year – increased to 50.5% in 2015 from 50.0% in 2014, the highest ever except for 2012,” Saez wrote. It should not come as a shock that to many Americans talk of economic recovery rings hollow. The top 1% of families saw their income grow by 37% between 2009 to 2015, from $990,000 to $1.36m. The incomes of the other 99%, however, grew by just 7.6% during that time – from $45,300 in 2009 to $48,800 in 2015. In 2015, the income of the 99% grew by just 3.9%. After factoring in inflation, Saez calls it: “the best real income growth in 17 years”. And the rich? At 7.7%, their growth was twice that. Economy remains a top concern for US voters, according to a recent Gallup survey of 1,530 adults. The gap between rich and poor is bigger now than it’s been just about any time since the 1920s.

The Chicago-based program Becoming A Man is the type that allows rival gang members to sit together, just days after one group killed a member of the other, and calmly talk about their issues, according to John Wolf, senior manager of the University of Chicago’s Crime Lab. “They were talking through ways of finding peace and ways of making sure it didn’t escalate further,” [Wolf said]. For the past few years, Wolf and his colleagues have been studying the impact of the Becoming A Man program, which targets at-risk male students in Chicago public schools. The program, run by the non-profit organization Youth Guidance, allows students to participate in weekly group sessions that teach them how to be more conscious of their decision-making processes. A recent two-year evaluation of the program showed that between 2013 and 2015, there was a 50 percent decline in violent crime arrests for the 2,000 participants as compared to a control group. The program does not tell students how to behave, or instruct them as to the “right” thing to do, instead [emphasizing] only that the students carefully consider their decisions instead of rushing to act. BAM says its approach is cost-effective: Every dollar invested in the program is projected to return up to $30 in societal gains as a result of crime reduction. Also, because the program increases graduation rates of participants by 19 percent, it will likely produce additional long-term economic gains.

Tailyn Wang was two months pregnant when federal police officers broke into her house in Mexico City, ripped off her nightgown and threw her to the ground. They groped her breasts while punching and kicking her in front of her terrified children, before taking her blindfolded to a police base – without an arrest warrant. Wang is one of scores of innocent women illegally arrested and tortured by Mexican security services looking to boost arrest figures to justify the war on drugs, according to damning new research by Amnesty International. Of the 100 women interviewed for the report, 72 said they were sexually abused during or soon after the arrest. Ten of the women were pregnant when arrested; eight subsequently suffered a miscarriage. The vast majority were young, poor, single mothers. Most spend years in prison awaiting trial, without access to adequate healthcare or legal advice. Wang, who has reported the torture to judges, prosecutors, doctors, and the National Commission for Human Rights, was falsely accused by an acquaintance, a local police officer, after he was also tortured. Reports of torture have increased exponentially in Mexico since former President Felipe Calderón first deployed tens of thousands of armed forces on the streets to combat warring drug cartels and organised crime. The navy, which has been deployed in some of the most violent states ... appears to have a particularly serious torture problem. If you had asked me 20 years ago where I would be, I never would have imagined I would be a physician working at UCLA Health, one of the best medical centers in the country. For over 25 years, my physical disability threatened to define who I was and what others thought I could become. I contracted meningococcal disease at 8 years of age. The infection overwhelmed my body's defenses, and I became a triple amputee. The disease left me with just enough to survive and carry on: two full fingers of the left hand, the thumb and ring finger. The first few years were physically and emotionally grueling; I was in and out of the hospital for surgical procedures to make my lower limbs fit better into prosthetic legs. I couldn't walk for nearly three years. I grew so quickly, my prosthetic legs could not keep up. My father would give me piggyback rides from the car to our house. My mother, who became blind as a teen, learned how to help me dress and put on my prosthetic legs every morning for school. My younger brother, Tarring, would help bring things to me since my mobility was limited. And my older sister, Nellie, was and is my inspiration and role model. I have been extremely lucky to have a strong and resilient family. I was lucky to be in a place where I had great medical care and where I had a community of friends and schools that supported my recovery and believed in my ability to succeed despite my disability. But luck is only part of my success; it takes courage, determination, honesty and integrity to pursue your dreams.

It is one of the world's biggest conspiracy theories – that the west was involved in plotting, organising, and even carrying out the 9/11 terror atrocities. Dr J Leroy Hulsey, chair of the University of Alaska Fairbanks’ (UAF) Civil and Environmental Engineering Department, has partnered with architects and engineers linked to campaign group 9/11 Truth to evaluate the causes of its collapse. WTC7 was not hit by a plane. However, it collapsed at 5:20 p.m. According to the National Institute of Standards and Technology (NIST), the official cause for the collapse was office fires. A growing number of family members, activists, architects and engineers question the official theory for collapse and are seeking a new investigation into WTC7. Dr Hulsey ... said: “NIST says the building fell down due to office fires. Our investigation will evaluate the probability that this was the cause of the collapse.”

If domestic abuse is one of the most underreported crimes, domestic abuse by police officers is virtually an invisible one. It is frighteningly difficult to track or prevent - and it has escaped America’s most recent awakening to the many ways in which some police misuse their considerable powers. It is nearly impossible to calculate the frequency of domestic crimes committed by police - not least because victims are often reluctant to seek help from their abuser’s colleagues. Courts can be perilous to navigate, too, since police intimately understand their workings and often have relationships with prosecutors and judges. Police are also some of the only people who know the confidential locations of shelters. Diane Wetendorf, a domestic violence counselor who wrote a handbook for women whose abusers work in law enforcement, believes they are among the most vulnerable victims in the country. Jonathan Blanks, a Cato Institute researcher who publishes a daily roundup of police misconduct, said that in the thousands of news reports he has compiled, domestic violence is “the most common violent crime for which police officers are arrested.” And yet most of the arrested officers appear to keep their jobs. An ABC 7 investigation this February found that nine of every 10 domestic violence allegations made against Chicago police officers by spouses or children resulted in no disciplinary action.

Pennsylvania’s Attorney General Kathleen Kane is used to going up against criminals, not her own colleagues. But after she helped expose an email scandal in the state government, her career has been in jeopardy. In 2014, Kane’s office released close to 400 pages of emails between state government employees, including some high-ranking state officials, exchanged on the state’s email server that were full of racism, sexism and pornography. The scandal was dubbed Porngate, and Kane said it involved a number of judges, prosecutors and law enforcement officials. “I couldn’t believe there was violence involved, and I couldn’t believe this wasn’t just some Playboy photos ... 398 pages, much of it hard-core porn,” Kane told “Nightline.” When she exposed the state email scandal ...Pennsylvania’s political good old boys club was furious [with Kane] and came after her. Amid calls for her resignation from Gov. Tom Wolf, the Montgomery County prosecutor charged Kane in August 2015 with perjury, obstruction of justice and criminal conspiracy. The state Supreme Court has suspended her law license over these charges, and she could face jail time. Kane said it’s revenge from those good old boys in the email scandal.

The Catholic church in Pennsylvania has been accused of employing “mafia-like” tactics in a campaign to put pressure on individual Catholic lawmakers who support state legislation that would give victims of sexual abuse more time to sue their abusers. The lobbying campaign ... is being led by Philadelphia archbishop Charles Chaput, a staunch conservative who recently created a stir after inadvertently sending an email to a state representative Jamie Santora, in which he accused the lawmaker of “betraying” the church and said Santora would suffer “consequences” for his support of the legislation. The email has infuriated some Catholic lawmakers, who say they voted their conscience in support of the legislation on behalf of sexual abuse victims. At stake ... is a state bill that would allow victims of sexual abuse to file civil claims against their abusers, and those who knew of abuse, until they are 50 years old. Critics of Chaput’s strategy say the archbishop used the same tactics to successfully derail similar legislation in Colorado, where he previously served as archbishop. Joan Fitz-Gerald, the former Democratic head of the state senate in Colorado who had introduced the bill, recalled it was the most vicious and difficult experience of her life, with Chaput allegedly telling one of his lobbyists that he did not believe Fitz-Gerald would be going to heaven. “He is the most vehement supporter of the secrecy of the Catholic church over pedophiles,” Fitz-Gerald said.

Cholesterol does not cause heart disease in the elderly and trying to reduce it with drugs like statins is a waste of time, an international group of experts has claimed. A review of research involving nearly 70,000 people found there was no link between what has traditionally been considered “bad” cholesterol and the premature deaths of over 60-year-olds from cardiovascular disease. Published in the BMJ Open journal, the new study found that 92 percent of people with a high cholesterol level lived longer. The authors have called for a re-evaluation of the guidelines for the prevention of cardiovascular disease and atherosclerosis, a hardening and narrowing of the arteries, because “the benefits from statin treatment have been exaggerated”. Co-author of the study Dr Malcolm Kendrick, an intermediate care GP, acknowledged the findings would cause controversy but defended them as “robust” and “thoroughly reviewed”. Vascular and endovascular surgery expert Professor Sherif Sultan from the University of Ireland, who also worked on the study, said cholesterol is one of the “most vital” molecules in the body and prevents infection, cancer, muscle pain and other conditions in elderly people. “Lowering cholesterol with medications for primary cardiovascular prevention in those aged over 60 is a total waste of time and resources, whereas altering your lifestyle is the single most important way to achieve a good quality of life,” he said.

Genentech and another drugmaker will pay $67 million to settle claims that they misled doctors into prescribing a treatment to lung cancer patients for whom the companies knew it would not work. As a result, some patients may have died earlier than they would have if they had taken more effective drugs, a lawsuit brought by a former Genentech employee and joined by federal prosecutors alleges. From 2006 to 2011 Genentech and its marketing partner OSI Pharmaceuticals promoted Tarceva to treat all patients with non-small-cell lung cancer even though studies had shown that it worked for just those who had never smoked or had a certain gene mutation known as EGFR. Epidermal growth factor receptor is a type of protein found on the surface of cells in the body. The whistle-blower lawsuit was filed in 2011 by Brian Shields, who worked as a Tarceva sales representative and then a product manager. The lawsuit said the companies ... discouraged doctors from testing patients for EGFR. The companies also promoted Tarceva ... by giving doctors illegal kickbacks disguised as fees for making speeches or serving on Genentech’s advisory boards. Sales representatives across the country were “instructed to spend lavishly” on physicians, the case said, and given “an unlimited budget to wine and dine.” Genentech also organized lunches or dinners for lung cancer patients where “patient ambassadors” were paid fees to speak about how Tarceva could be used in ways never approved by regulators, the lawsuit said.

The F.B.I. has significantly increased its use of stings in terrorism cases, employing agents and informants to pose as jihadists, bomb makers, gun dealers or online “friends” in hundreds of investigations. Undercover operations, once seen as a last resort, are now used in about two of every three prosecutions involving people suspected of supporting the Islamic State, a sharp rise in the span of just two years. F.B.I. operatives coax suspects into saying and doing things that they might not otherwise do - the essence of entrapment. “They’re manufacturing terrorism cases,” said Michael German, a former undercover agent with the F.B.I.. Despite dozens of challenges to undercover terrorism cases brought since the Sept. 11, 2001, attacks, a judge has yet to throw one out on the grounds of entrapment. Not that some judges have not considered it. “I believe beyond a shadow of a doubt that there would have been no crime here, except the government instigated it, planned it and brought it to fruition,” Judge Colleen McMahon of the United States District Court ... said in 2011 in a case involving four Muslim men in Newburgh, N.Y. The F.B.I. planted an undercover informant inside a mosque in Newburgh as part of what became an elaborate, nearly yearlong plot. The F.B.I. even built a fake Stinger missile and had it delivered to the men. Judge McMahon said she was troubled by the F.B.I.’s conduct, but she upheld the charges.

Prime ministers, finance ministers, leading entrepreneurs and a former spy chief are among the attendees at this year’s influential Bilderberg conference ... which begins on Thursday in Dresden. The German military has been drafted in to oversee security. They’re working with corporate security from Airbus to make sure the politicians are kept safely away from the press for the entire three-day conference. Every year, a major corporation with links to the Bilderberg steering committee coordinates security for the event with the police. Which makes the whole conference even more obviously the corporate lobbying event that it is - with giant corporations handling everything from security to dry ice. And it makes the silence of the politicians who attend even more egregious. These high-level talks between policymakers and the heads of transnational oil companies ... take place in heavily guarded privacy, with no press oversight whatsoever. [This is] especially great if you’re on the board of BP. Like, for example, Sir John Sawers. As well as being a director of BP, the silken, Blairish former MI6 boss is a member of Bilderberg’s steering committee, and the chairman of Macro Advisory Partners, a global advisory group with heavy links to the transatlantic intelligence community, very much in the style of Kissinger Associates. And speak of the devil! The ageless 93-year-old former US secretary of state will be holding court at Dresden, croaking out his wisdom from the throne of bones he has shipped everywhere he goes.

The bidding opens at $9,000. For sale? A Yazidi girl. She is said to be beautiful, hardworking, and a virgin. She's also just 11 years old. This advertisement – a screengrab from an online marketplace used by ISIS fighters to barter for sex slaves – is one of many Abdullah Shrem keeps in his phone. Each offers vital clues – photographs, locations – that he hopes will help him save Yazidi girls and young women like this girl from the militants holding them captive. Shrem was a successful businessman ... when ISIS came and kidnapped more than 50 members of his family from Iraq's Sinjar province. Tens of thousands of Yazidis fled their homes ... while thousands of women and girls were abducted and sold into slavery. Desperate – and angered at what he saw as a lack of support from the international community – he began plotting to save them himself, recruiting cigarette smugglers used to sneaking illicit produce in and out of ISIS territory to help his efforts. So far, he says, his network has freed 240 Yazidis; it hasn't been easy, or cheap – he's almost broke, having spent his savings paying smuggling fees. A number of the smugglers have been captured and executed by ISIS while trying to track down Yazidi slaves. But Shrem insists the risks are worth it: "Whenever I save someone, it gives me strength and it gives me faith to keep going until I have been able to save them all." Once they manage to make contact ... it can take days or even weeks to get safely out of ISIS territory.
