In general, we rely on organizations like the CDC to conduct surveillance and monitoring of diseases in the United States. Unfortunately, case reporting, which we are relying on now to understand COVID-19 in the United States is the weakest type of surveillance for an ongoing pandemic. Case counts meant something very early on in the epidemic when each case reliably was associated with a certain risk of severe disease, hospitalization or death. Once there was a substantial number of people with immunity to severe outcomes due to recovery or vaccination those case counts became disconnected from expected outcomes. Test positivity also used to be a reliable measure of the community burden of infection with increased positivity correlating with increased spread of infection and hospitalization. But because now many only seek medical testing to confirm a home-based positive test for employment sick-leave or other purposes, the frequency of those testing positive through medical testing is artificially high. Remember the days of "flatten the curve," the idea that we had to preserve hospital capacity through efforts to reduce the spread of infection? That curve was the number of hospital admissions due to COVID-19 and reasonably reflected the number of people admitted to hospitals severely ill with COVID-19. Now, however, due to the continued universal screening of all hospital admissions, a majority of reported COVID-19 hospitalizations are not hospitalized "for" COVID-19 but "with" COVID-19.

When Paul McCartney wrote "Get Back," he never would have predicted how useful or relevant the song would become for music therapists. In new research, Psyche Loui, an associate professor of music ... found that for older adults who listened to some of their favorite music, including The Beatles, connectivity in the brain increased. Specifically, Loui–and her multi-disciplinary team ... discovered that music bridged the gap between the brain's auditory system and reward system, the area that governs motivation. "There's something about music that is this functional connectivity between the auditory and reward system, and that's why music is so special and able to tap into these seemingly very general cognitive functions that are suddenly very engaged in folks with dementia who are hearing music," said Loui. The original idea for this research came out of Loui's own experiences playing music in nursing homes. She recalled how people who couldn't finish a sentence or thought would suddenly harmonize and sing along to a song she was playing. "[Music] seems to engage the brain in this way that's different than everything else," Loui said. What the researchers found was striking: Music was essentially creating an auditory channel directly to the medial prefrontal cortex, the brain's reward center. Music that was both familiar and well-liked tended to activate the auditory and reward areas more. The music that participants selected themselves provided an even stronger connection.

"It's like a horror movie I'm being forced to watch and I can't close my eyes," one senior FDA official lamented. Spain has announced it will make some short- and middle-distance train journeys completely free as the government seeks to combat the impact of the cost-of-living crisis. Spanish prime minister Pedro Sánchez said on Tuesday that all CercanĂas (commuter trains), Rodalies (commuter routes in Catalonia) and Media Distancia routes (mid-distance regional lines which cover distances less than 300km) run by the national rail system will be free from 1 September 1 to 31 December this year. The 100% discount will only apply to multi-trip tickets. The new scheme comes in addition to the government funding a discount of between 30-50% on all public transport - including metros, buses and trams. Spanish inflation has accelerated in the past few months and surpassed 10% for the first time in 37 years during the 12 months to June. Sanchez said soaring inflation was the biggest challenge for Spain, likening it to "a serious illness of our economy that impoverishes everyone, especially the most vulnerable groups". A new wave of anger swept through Uvalde on Tuesday over surveillance footage of police officers in body armor milling in the hallway of Robb Elementary School while a gunman carried out a massacre inside a fourth-grade classroom where 19 children and two teachers were killed. The video published Tuesday by the Austin American-Statesman is a disturbing 80-minute recording of what has been known for weeks now about one of the deadliest school shootings in U.S. history: that heavily armed police officers, some armed with rifles and bulletproof shields, massed in the hallway and waited more than an hour before going inside and stopping the May 24 slayings. The footage from a hallway camera inside the school shows the gunman entering the building with an AR-15 style rifle and includes 911 tape of a teacher screaming, "Get down! Get in your rooms! Get in your rooms!" Two officers approach the classrooms minutes after the gunman enters, then run back amid the sounds of gunfire. As the gunman first approaches the classrooms a child whose image is blurred can be seen poking their head around a corner down the hallway and then running back while shots ring out.

A former Boston police officer who was beaten more than 25 years ago by colleagues who mistook him for a shooting suspect will be the new leader of the city's police department, Mayor Michelle Wu announced. Michael Cox, 57, will return to his hometown of Boston after working as the police chief in Ann Arbor, Michigan, to lead the same force he once brought a civil rights case against over his beating by fellow cops. Cox, who is Black, will take over as commissioner next month. Before becoming chief in Ann Arbor in 2019, Cox was part of the Boston police force for 30 years, where he rose through the ranks after fighting for years to get justice over his beating that left him seriously injured. Cox was working undercover in plainclothes as part of the gang unit in January 1995 when officers got a call about a shooting. Cox, dressed in jeans and a sweatshirt, spotted the suspect. The suspect started to scale a fence and Cox was struck from behind just as he was about to grab the man. He was kicked and punched by fellow officers, suffering head injuries and kidney damage. Cox has described facing harassment in an effort to silence him after the beating became public despite efforts by his colleagues to cover it up. A department injury report said Cox lost his footing on a frozen puddle, causing him to fall and crack his head. Cox chose to stay in the police force after what happened to him and try to improve things instead of walking away from a job he loved.

John Bolton, a former U.S. ambassador to the United Nations and former White House national security adviser, said on Tuesday that he had helped plan attempted coups in foreign countries. Bolton made the remarks to CNN after the day's congressional hearing into the Jan 6, 2021, attack on the U.S. Capitol. The panel's lawmakers ... accused former President Donald Trump of inciting the violence in a last-ditch bid to remain in power after losing the 2020 election. Speaking to CNN anchor Jake Tapper, however, Bolton suggested Trump was not competent enough to pull off a "carefully planned coup d'etat," later adding: "As somebody who has helped plan coups d'etat - not here but you know (in) other places - it takes a lot of work. "I feel like there's other stuff you're not telling me (beyond Venezuela)," the CNN anchor said, prompting a reply from Bolton: "I'm sure there is." Among the more than 1.2 million Americans imprisoned in federal and state prisons, two out of three are forced to work while imprisoned. The 13th amendment of the US constitution abolished slavery or involuntary servitude, but included an exception for prisoners; critics have called prison work modern-day slavery. [Susan] Dokken's pay started at 12 cents an hour and prisoners have the ability after positive reviews to increase their pay to 24 cents an hour, while they're charged full price when they buy basic necessities through the commissary. Dokken explained that if prisoners refused to work, they would have privileges revoked and possibly get written up, which would follow them on their record to parole and probation. According to a June 2022 report published by the American Civil Liberties Union, prison labor generates more than $11bn annually, with more than $2bn generated from the production of goods, and more than $9bn generated through prison maintenance services. Wages range on average from 13 cents to 52 cents per hour, but many prisoners are paid nothing at all, and their low wages are subject to various deductions. The ACLU report said 76% of workers surveyed reported they were forced to work or faced additional punishment, 70% said they could not afford basic necessities on their prison labor wages, 70% reported receiving no formal job training and 64% reported concerns for their safety on the job. Prison workers are also excluded from basic worker protections.

Bashing Saudi Arabia during a presidential election season is almost a tradition in the United States, and President Biden made no exception. The United States needs Saudi Arabia: The kingdom remains the oil market's major swing producer and is the main buyer of U.S. arms globally. By virtue of geopolitics and economics, Saudi Arabia's cooperation with the United States is consequential when it comes to Washington's efforts to counter Iran, end the war in Yemen and normalize Israel's relations with the Arab world, as well as limit Russia's and China's influence in the region. All of this was true before Russia's invasion of Ukraine upended global oil markets and sent gasoline prices skyrocketing in the United States and Europe. So the Biden administration had to come up with a solution to its Saudi problem, especially in a critical election year, as Mr. Biden's job approval ratings have dropped and gas prices have soared.

We found internal emails from these companies where the people in the companies were laughing at the addicts. They were mocking them. Meanwhile, the drug companies, they are smart. They decide to lure away the best and the brightest if they can from the DEA and the Justice Department to help them as they are selling opioids, and they are very successful. They hired dozens of people from DEA and the Justice Department to work for these companies. So again, these are the people who are trying to protect us, working for the DEA and the Justice Department. They are lured away to the companies who are selling addictive painkillers that are killing people."

Mayor Eric Adams defended New York City's newest PSA on Tuesday, saying a nuclear attack preparedness spot from the Office of Emergency Management was a "great idea" born out of the ongoing Russian war in Ukraine. The campaign launched Monday and features a short PSA outlining three steps that New Yorkers can take "as the threat landscape continues to evolve." There are no imminent nuclear threats to New York City, Adams emphasized, but there will be a series of emergency management ads highlighting preparedness efforts. California will begin making its own low-cost insulin in an effort to make the essential diabetes treatment more affordable, Gov. Gavin Newsom announced on Thursday. "Nothing epitomizes market failures more than the cost of insulin," the governor said in a video posted on Twitter, "Many Americans experience out-of-pocket costs anywhere from three hundred to five hundred dollars per month for this life-saving drug." With a budget of $100 million, California plans to "contract and make our own insulin at a cheaper price, close to at cost, and to make it available to all," Newsom said. It's unclear exactly how inexpensive California's insulin will be or when the low-cost drugs will be available. Insulin in the U.S. costs almost $100 per unit, on average. That's nearly four times the price in Chile, which has the second-highest prices among the 34 countries analyzed by the nonprofit Rand Corporation, at less than $25 per unit. Currently, four in five Americans in need of insulin have incurred thousands of dollars in credit card debt to pay for the medication, according to a recent survey commissioned by health care organization CharityRx. The average debt among all survey participants was $9,000. California's program will allot $50 million toward the development of cheaper insulin products and $50 million on an in-state insulin manufacturing facility, Newsom said, adding that the facility "will provide new, high-paying jobs and a stronger supply chain for the drugs."
